In Office Visits​

Please arrive at least 10 minutes prior to your appointment time to allow sufficient time to complete any co-payments or paperwork necessary. If you arrive for your appointment more than 15 minutes late, please be aware that your appointment may be rescheduled, as to not inconvenience the next scheduled patient.
Initial intake paperwork will be emailed to you via our client portal. All paperwork must be completed prior to our administrative staff verifying your insurance benefits, and scheduling your intake appointment. If you need further assistance, please let our staff know by calling 571-636-1800.

If you cannot keep your appointment, please call 571-636-1800 to cancel or reschedule your appointment AT LEAST TWENTY FOUR (24) HOURS IN ADVANCE! Failure to do so may limit your ability to be seen at your preferred slot or with your preferred provider in the future. Established clients may be charged a no-show fee in accordance with the policies and procedures.

WHAT TO BRING TO YOUR APPOINTMENT

  • Valid Photo ID
  • Insurance Card
  • List of current prescriptions and/or over-the-counter Medication, including dose and frequency
  • Information about your medical history or other relevant information
  • Payment is run via Ivy Pay, please bring your cell phone with you, or bring cash to the appointment.

TeleHealth Visit​

What is a telehealth visit?

A telehealth visit is the use of digital information and communication technologies, such as computers and mobile devices, to access health care services remotely and manage your health care. In short- it’s a video chat with your provider through your phone, tablet, or computer from the comfort of your home!

How does it work?

You call our office or email our admin staff at [email protected] and request an appointment. We will gather your information from you with the best way to reach you for the video chat.

You will email us a picture of your Insurance Card (front and back) and your driver’s license.

You will get an email with a link to our patient portal. Once in the patient portal you will fill out the patient documents under intake documents.

You will be asked to pay your copay at the time of your intake appointment

At the time of your scheduled appointment you will get a text message or email (whichever platform you have designated to do your telehealth visit through) from the provider you are seeing that will have a link attached to it.

You will click the link at the time of your appointment.

After clicking the link, the window to the online portal will open, and it will ask for you to select your camera and microphone, as well as enter your name. For your initial appointment, please write your full name.

After that you will be connected with the provider and the video visit should connect momentarily.

GUIDELINES FOR TELEHEALTH APPOINTMENTS

*We ask that you find a quiet place away from distractions to have your visit.

*Please ensure that both camera and microphone are active, and that you have a solid internet connection.

*Absolutely NO DRIVING during your telehealth visit!

Insurances Accepted & Payment

Our providers participate with most major insurance plans such as Medicaid, Tricare, Anthem, Optum, Aetna, Cigna, and United Healthcare. We also accept referrals from the Veteran’s Administration (VA), and can help Veterans with questions gain access to this care. We verify benefits prior to your appointment, and will provide the co-pay or deductible information upon request. We also provide a self-pay rate for those who are uninsured. Please call our office at 571-636-1800 and we will gladly answer any questions you have. Payment is expected at time of service. If you are experiencing financial issues, please let us know prior to your appointment.
